As you browse through the PlayStation Now games, you’ll see a tag in the game preview section telling you if the game is downloadable. To download an eligible game, select the game and select Download.
You can access a downloaded PS Now game via the PS Now app or directly from your game library:

Open the app > Browse Games > select the game > Start Game.
Go to your Games home > Game Library > PlayStation Now > select the game to start.

Yes. You can stream games that you’ve downloaded by launching them through the PS Now app on PS5, PS4 or PC. To keep your existing game progress, you’ll need to transfer your latest saved game data from your downloaded game to the PS Now streaming service.
You can purchase and use any DLC and add-on content from the PlayStation™Store with games downloaded from PlayStation Now. If your PlayStation Now subscription expires, you’ll lose access to both the downloaded game and any DLC or add-on content associated with it. Re-subscribe to PlayStation Now or purchase the game to be able to use the DLC and add-on content.
Games downloaded via PS Now will allow you to use any peripherals that the game supports.
