Yes. Phantom Blade Zero initially offers three difficulty settings — Wayfarer, Pathbreaker and Hellwalker — roughly corresponding to Easy, Normal and Very Hard, and there is a fourth, special unlockable mode called 66 Days.

In 66 Days, you play at Hellwalker difficulty under the story's core premise: Soul has only 66 days left to live. Every death deducts one day — if all 66 days run out, Soul permanently dies and the playthrough ends.
