Error Code SU-41350-3 (it's not in the ec directory)

So recently I've been getting an error code SU-41350-3. It doesn't appear in the error code directory and here are the steps I've gone through to trouble shoot.

Start the machine, machine tries to start but can't and goes into safe mode. Tells me to connect DS4 to USB cable and press PS button, then it tells me to connect a USB storage device that contains an update file for re-installation for version 5.00, which I have set up. It then tells me please wait and says "Cannot Find Update file." and gives the error code.

Like I said, the code doesn't appear in the directory.

The flash drive needs to be either FAT32 or exFAT format

On the drive you need a folder called "PS4". Inside that folder you need "UPDATE". Then the file needs to be in that folder, and called "PS4UPDATE.PUP" (file and folder names must be in uppercase like that)

It possible that you might need the larger (900MB) installation file instead of the smaller update file. Unfortunately you need to initialise the system to use this, which will delete everything. But try the update file first to avoid deleting everything

Man, I'm having the exact same problem!! I've tried flash drive way several times and nothing has worked. Now this FAT32 or exFAT is something I haven't tried so I will look into that. But, I've talked to Sony Support 3 times in the past week and they haven't been any help. THEY CAN'T EVEN EXPLAIN WHAT THE HELL IS GOIN ON!! Today was the last time I called them and it was weird because I was told to do the same thing again. I told the guy nothing's working and the he told me to select #6 on the safe mode menu. Of course, I told him that would erase everything and after I said that he quickly told me that will fix it and if I have anymore questions to call them back. This dude couldn't get off the phone fast enough after I said that #6 would erase everything. Sony is being very shady and unwilling to help bor explain what's going on!! I would initialize my PS4 since I have everything backed up and I'm not at risk of losing anything but I want to know what the hell is goin on before I do that. What if option 6 doesn't work? Anyway, sorry for venting. If I get mine going, I will let you know what I did to fix it. Could you please do the same for me?

So the fix is initialize the PS4. After trying multiple ports and multiple drives, both empty and known-good (all formated in FAT32), Initialization was the only fix.
